While wo don't enjoy paperwork as a general rule, we do want make certain that solid communication takes place. Thus, we will post any and all of the forms sent home with the students. If a student misplaces a field trip form, it should be here. If an emergency form still needs to be completed and returned, it should be posted here. If we forgot a form (or two), let us know and we'll post it, too.
Welcome Packet Mailer Forms
The official Welcome Packets will be available during Orientation 2010 and all of the general forms can be downloaded...just in case you need an extra copy or if we neglected to include a copy of a particular form. This form should be completed twice per year--once at the beginning of the school year and once at the beginning of the second semester. Completed forms should be returned to the main office or emailed to Ms. Gold at clgold@cps.edu.
Attached is the CPS Minimum Health Requirements for freshmen during the 2010-2011 school year. Physical Examination Deadline: October 1, 2010. This document is in English and Spanish.
